Dangerous loose concrete in laneway

At the back of 66 Giles street Kingston in the laneway near the Supabarn loading dock door there is a huge hole in the road with a piece of concrete that is raised and loose. This is dangerous and when a car drives over it the concrete moves. Just worried someone is going to be injured by this. Please repair. Thank you.
